Three Important Ways we Continue to Serve You
We believe that having a strong discharge plan is a marker for success, so we help alumni feel confident with their future plan as they take that step forward. Prior to leaving our program, each patient meets with our Alumni Coordinator for a transition plan meeting to discuss their progress and experience in the program. We also identify needs for outpatient resources. In the planning meetings, alumni reflect on their goals for the next chapter of their lives and identify areas where they may need support after leaving the program.
Our Alumni Coordinator, together with our Clinical Outreach Professional and your Care Team, cultivate a list of potential referrals and resources patients may contact to build their outpatient team.

Sometimes, alumni need just a little bit of encouragement to gather the courage to take steps to integrate back into life. That is why our Alumni Coordinator is available for support calls once patients leave our program. A support call can bridge the gap and remind alumni of what they learned during their time at Lido Wellness Center that may be beneficial on their unique path to recovery.
A Lido Wellness Center alumni can anticipate to hear from us at 30, 60, and 90 days after they depart. These check-in calls give alumni the opportunity to discuss progress on the goals they set during the Exit Interview. It is a good time for alumni to assess how they are integrating and managing their lives post-treatment.
